The Australian Transport Safety Bureau (ATSB) released the report of its investigation into the 21 August 2003 collision between the bulk carrier LANCELOT and the fishing vessel JENABAR off Diamond Head, New South Wales. The primary cause of the incident was found to be a failure on both vessels to maintain an adequate lookout. A secondary cause was over-reliance by the bulk-carrier on information from the automatic radar plotting aid (ARPA). Marine Safety Investigation No. 196
